Creating Checkboxes

By creating checkboxes, you are giving your visitors the option to select multiple items.  To do this, use the information provided in the example below to create checkboxes.

Example

Suppose you would like to add checkboxes so a visitor can select their favorite color, below is the code you would enter to do this:

Favorite Color?
<input type="checkbox" name="red"> Red

<input type="checkbox" name="blue"> Blue
<input type="checkbox" name="green"> Green
<input type="checkbox" name="other"> Other

Note: The input name (i.g."red", "blue". "green", etc.), may not have spaces in it.  Please use the underscore "_" character instead, or make it one word.

 Note: For user@domain.com, you will need to enter the email address where the form will be sent to.
